Thank you for your interest in The Carter Academy. We’re honored to become a part of your child’s early learning experiences—and we’re excited to get to know you, your family members, and the other important people in your child’s life. 

TIPS FOR COMPLETING FORMS:

• Forms will need to be PRINTED out. Original signatures are required on all documents.
• Use a separate form for each child 
• Fill out ALL sections on each form
